Dear Sir/Madam,

We experienced an issue processing your recent purchase and we require some further verification in order for us to set up the xxxx account. We apologize for any inconvenience this may cause, but it is important to us that we protect our customers from the fraudulent use of personal information.

If you are paying with a credit card, please send us a copy of the front of the credit card and the card holder's passport or driver's license to confirm the payment. Please mask any of the details that you wish to hide, however, please make certain that the name on the credit card and the last 4 digits of the card number are still viewable.

If you are paying with a PayPal account please email us a form of ID for the owner of the PayPal account, such as your passport or driver's license.

There are 3 easy ways to complete the verification process:

-You can scan the information and email it to us at compliance@ipage-inc.com.

-You can take a photo of the information with your mobile phone and email it to us at compliance@ipage-inc.com.

-You can fax a copy of the information to 781-998-8340.
